A. Financial responsibility is based on the type of insurance coverage you have. Prior to the start of therapy, a Chartwell Insurance Verification Coordinator discussed any potential financial responsibility and payment options with you. If you have any questions regarding financial responsibility, please contact your team Insurance Verification Coordinator.
Q. I received a bill from Chartwell and have questions, who do I speak to?

A. A Chartwell Insurance Verification Coordinator spoke with you prior to starting services to discuss how your insurance will pay for your therapy. Please contact your Insurance Verification Coordinator if you have any further questions.
Q. What is this financial responsibility form that I am required to sign?
A. Chartwell's financial responsibility acknowledgement form reflects the information we received from your insurance plan regarding covered benefits. We also list an estimate of the cost of our services and your estimated out of pocket expenses. Signing the acknowledgement letter shows that you understand how our services will be paid for and that you understand your financial responsibility to Chartwell.
